Go to Setup from main menu and click on Provider
From the Provider List click on the Edit 
button corresponding to the provider that needs to be assigned to multiple Legal entity/ Multiple locations
From Provider Legal Entity list select the legal entities that are to be assigned to the provider. To make multiple selection click on the Ctrl button on the keyboard and select the legal entity from the list. Click on Save after selecting the Legal Entities
